Canada's famous Quebec Bridge / Pont du Quebec, became known as the Bridge the Fell Twice, and cost the lives of 89 men.

The Quebec Bridge

The famous 'Quebec Bridge' collapse caused 76 deaths of steel workers due to a calculation and design error discovered but not reported by the chief engineer. The bridge's second collapse was caused by the use of poor steel, causing thirteen more deaths. Today engineers wear iron rings to remind them of this and other spectacular failures.
